We serve people of all ages, backgrounds, languages and identities. Eligibility is set by the member’s health plan and the State.

- Individuals and families experiencing homelessness or at risk of it
- Older adults and people with disabilities who need long-term supports
- Adults leaving hospitals, skilled nursing or institutional settings
- People returning from incarceration
- Members with serious mental illness or substance use needs
- Youth and young adults, including those leaving foster care
- Pregnant and postpartum members
- Veterans
- Members who need a licensed residential care placement
- Members who need in-home help to stay housed

We provide services in the member’s preferred language, with interpreter access.
Disability access
We meet people where they are, including at home, shelters and clinics.
Cultural humility
We listen first and ask about identity and preferences without assuming.
No cost to members
Community Supports are a Medi-Cal benefit. Members never pay Hestia Crest.
