Care Manager
Support members from referral through move-in and stabilization, document services, and coordinate with plans and providers.
Looking for
- Experience in case management, social services or community work, or relevant lived experience
- Strong communication and documentation skills
- Reliable transportation and a willingness to meet members in the community
- Bilingual skills a plus (English, French and Swahili are especially welcome)
